The RN Case Manager oversees medical support services for individuals in a 24/7 LPN-staffed home and those needing nursing intervention in non-medical model homes. Responsibilities include developing Nursing Care Plans, training staff on emergency protocols and nursing tasks, ensuring tasks are performed safely and in compliance with standards, managing delegated tasks, monitoring medical needs, advocating for individuals’ health, and liaising with healthcare providers.
