Location: Yreka, CA- Onsite. Patient Population: Older adults ages 65 and up. Salary: up to $106,000 per year. $5K SIGN-ON BONUS & HOUSING ASSISTANCE. New program opening in May 2025. Lead disciplinary team providing therapy to seniors. Engage with community, develop referrals, maintain patient census. Ensure compliance, other duties as assigned.
