The Pediatric Care Coordinator works within a primary care team to promote timely access to care, enhance communication among providers and families, assess child and family needs, develop care plans, coordinate services, and support quality improvement efforts. Requirements include a Bachelor’s Degree in a related field, 3 years of relevant experience, leadership and communication skills, and proficiency in computer use. Physical abilities include standing, walking, sitting, and using office equipment. The work environment is typically quiet with moderate exposure to blood borne pathogens.
