The RN Case Manager at this hospice care facility will plan, organize, and direct hospice care emphasizing community health education/experience. Responsibilities include assessing patient and family needs, maintaining accurate clinical notes, and traveling throughout the day and sometimes after-hours. On-call schedule required. Required: RN licensure, clean driving record, valid driver’s license, and auto insurance.
