Responsibilities include patient assessment, care planning, patient and family support, case management, and hands-on care. Ideal candidates have 1-2 years of acute care and home care experience, a valid nursing license, CPR certification, and strong communication skills. Physical requirements include standing, walking, sitting for prolonged periods, lifting patients and supplies, and specific vision and hearing abilities.
