Qualifications: Physical Therapy License or Physical Therapy Assistant License (Required), CPR Certification (Preferred). Pay: Physical Therapist: $42.00 – $60.00 per hour DOE, Physical Therapy Assistant: $30.00 – $35.00 per hour DOE. Primary Responsibilities: Provides physical therapy to home care patients, case management, communication, documentation, and service development. Specific Job Duties: Evaluate and treat patients, assessment, care planning, collaboration, monitoring, patient education. Work Location: In person, full-time or part-time, day shift, Monday to Friday, no weekends.
