Provides direct integrated co-occurring behavioral health and substance use services in support of integrated primary care. Conducts screenings and assessments, creates treatment plans, and provides referrals. Communicates with medical providers and participates in system change. Ensures compliance with documentation standards and utilizes Electronic Health Record. Must have a Master’s Degree in relevant field and state licensure. Preferred experience in outpatient mental health and/or substance use services. Multi-lingual skills are a plus.
