Responsible for managing and integrating Care Coordination services within a health system. Provides person-centered acuity-based care coordination in hospital or clinic settings. Collaborates with staff to deliver high-quality services, promotes person-centered care, and assists in protocol development. Requires RN, BSN, NM Nursing License, 2+ years healthcare experience, and knowledge of health care facilities and services. Reports to Manager of Population Health and Director of Ambulatory Patient Care Services. Works in various care settings, requiring good vision, dexterity, and customer service skills.
