Responsible for providing nursing care to patients in a home health setting. Requires RN license, BSN preferred, and 2 years of acute care experience. Must be able to perform patient care tasks and work in various environmental conditions. Responsibilities include assessment, case management, after-hours care, developing care plans, supervising staff, and collaborating with other disciplines. Must demonstrate critical thinking, effective communication, and promote patient safety and education.
