Provides case management for assigned patient populations, utilizing clinical expertise to achieve optimal outcomes. Promotes cost-effective care, interdisciplinary collaboration, and appropriate length of stay. Collaborates with Utilization Review Nurse, coordinates care, and performs patient assessments. Ensures smooth transitions, advocates for patients, and participates in performance improvement initiatives. Requires a Registered Nurse with a Bachelor’s degree and 3 years of clinical healthcare experience. Must have current Virginia RN licensure and demonstrate various competencies.
