Position Summary: Works with the Home Care Director and Home Care Manager to ensure effective coordination and delivery of PACE home care services. Provides direct supervision to health aides, conducts audits, monitors schedules, participates in long-range planning, and serves as a resource for staff and participants. Assists in policy review and revision, oversees record documentation, and identifies opportunities for improvement. Qualifications include a BS in Nursing, current MA Nursing License, strong clinical background in geriatric care, and expertise in dealing with elders and families.
